Reducing Risk and PCC – Christine Vanderley-Reichner

Often in Medical Imaging we separate out quality from the patient experience and focus on “getting the images”. In my time working as a safety and quality supervisor and previously as a CT supervisor I have come across lots of scenarios that would have been improved by better communication, more listening to the patient and increased communication among team members.
The examples that I am going to use today have all resulted in unfortunate outcomes for the patient or images that were sub optimal. Some have even left imaging staff scarred for life- we could call it PTSD. We need to constantly ask ourselves, did my performance meet the “pub test” and a higher standard could be ” what if this was my mother/father”. Simple communication, documentation, protocols and following good risk-averse procedures would have made all the difference.
